On February 12, 2016, about 0745 Alaska Standard Time (AKS), a Lockheed 382G airplane, N401LC, had a rapid depressurization during cruise flight about 100 nautical miles east of Bethel, Alaska. The flight crew declared an emergency and diverted to Iliamna Airport, Iliamna, Alaska, where they landed without incident. There were no injuries to the crew. The airplane sustained substantial damage. The aircraft was registered to and operated by Lynden Air Cargo, LLC, under the provisions of 14 Code of Federal Regulations Part 121 Supplemental as a scheduled domestic cargo flight. Visual meteorological conditions prevailed for the flight, which operated on an instrument flight rules flight plan. The flight originated from Bethel Airport (PABE), Bethel, Alaska, at 0650 AKS and was enroute to Elmendorf Air Force Base (PAED), Anchorage, Alaska.

Examination of the airplane revealed a large hole in the forward pressure bulkhead canted web.
